Information for Patients

If your scan relates to a Workers’ Compensation claim, please ensure you have the following information available:

A valid referral from your treating doctor
Your Workers’ Compensation claim number (if issued)
The name of your insurer
The date of your injury

Once written approval has been obtained from your insurer, approved imaging services are typically provided with no out-of-pocket expense.
